ABSTRACT

Objective:To investigate the clinic effect of r-MHT and hematoma aspiration on the traumatic intracerebral hematoma.Methods:89 cases with traumatic intracerebral hematoma were given hematoma aspiration,47 of them were given r-MHT and hematoma aspiration,the clinic effect on the 1st,7th,14th day after treatment were evaluated by NIHSS,the hematoma volume before treatment on the 1st,7th,14th day after treatment were counted by Dotian formula.Results:The effective rate of treatment group was 93.6%,which was significantly higher than that of the control group (P<0.05).The NIHSS score of treatment group was significantly higher than that of the control group(P<0.05) on the 1 st day,1st,2nd week after treatment (P<0.05).Conclusion:r-MHT and hematoma aspiration couldn effectively reduce the brain damage,improve the patient's neurological fumction in treating traumatic intracerebral hematoma.

ABSTRACT

Objective:To observe the effect of stereotactic aspiration combined with corresponding drug therapy in the treatment of cerebral hemorrhage induced by different drugs. Methods:Totally 66 patients with hemorrhage induced by antithrombotic drugs were randomly divided into the control group and the treatment group with 33 cases in each. The control group was given corresponding drug therapy according to the pathogenesis of cerebral hemorrhage, and the treatment group was treated by stereotactic hematoma aspiration additionally. Daily life ability, NIHSS and prognosis improvement were compared between the two groups before and after the treat-ment. Results:After the treatment, NIHSS of the treatment group was ( 10. 17 ± 2. 97 ) and Barthel index score was ( 59. 97 ± 12. 10), which were both better than those of the control group (P<0. 05). The prognosis of the treatment group was also better than that of the control group (P<0. 05). There was no severe adverse drug reaction in the two groups. Conclusion:For different drug in-duced cerebral hemorrhage, corresponding drug therapy should be carried out, and combining with stereotactic hematoma aspiration, the therapeutic regimen can effectively improve neurological deficits and activities of daily living of the patients, and then improve the prognosis of the patients with promising safety.

ABSTRACT

Objective To evaluatethe application value of APACHE Ⅱ on the prognoses in prediction of pin hole multi-directional intracranial hematoma aspiration to cure severe cerebral hemorrhage.Methods APACHE Ⅱ scoring system was conducted in randomly collected 58 severe cerebral hemorrhage patients' data (30 patients underwent surgical treatment,28 patients underwent conservative treatment)to evaluate the disease severity,prognosis and effect of the pin hole multi-directional intracranial hematoma aspiration.Results The APACHE Ⅱ score of treatment group on the first day was 28.00 ± 1.92 and the predicted mortality was 0.86 ±0.03.The score was decreased obviously on the tenth day(8.20 ± 0.76)and the predicted mortality was 0.57 ± 0.05.The actual mortality was 33.33%.Conclusion APACHE Ⅱ scoring system is suitable for judging the disease severity and predicting the prognosis of severe cerebral hemorrhage patients,it proves that surgical treatment effect of the pin hole multi-directional intracranial hematoma aspiration is remarkable.

ABSTRACT

6 patients underwnet CT-guided stereotactic biopsy or hematoma aspiration procedure for posterior fossa lesions. Trajectory for biopsy or aspiration was transcerebellar approach. The patients were placed under general endotracheal anesthesia and positioned prone or sitting. No complications were encountered in the postoperative period when this technique was used. Stereotaxic transcerebellar multiple biopsy can safely provide tissue conformation of the neoplastic nature of a posterior fossa lesion thought to be a tumor. In addition, hematoma aspiration has been demonstrated to be safe, reliable, and efficient.
